To register for an upcoming EIPA, please download the registration form for the date of the exam you wish to take. Complete the registration form and demographic information (leaving questions 3-6 blank) then mail it, with your payment of $250 (made payable to TAESE/USU) to: EIPA Registration, ASK12 1780 North Research Parkway Suite 112 North Logan, UT 84341 Or you can fax your registration to 435-753-9750. Registration closes 30 days prior to the testing date. You will receive a letter and email from ASK12 confirming your registration, informing you of your test time, and giving you other information about the exam. Note: If you do not submit the demographic information, payment or contact information for who will be paying, or if you do not circle your evaluation material, your registration will not be accepted.
