IDEA 2004 required states to implement most changes to the IEP in July 2005. Following are some different states' approaches to providing IEP information and forms to their districts.

Connecticut IEP Manual and Forms
This document provides guidance and direction on the development of IEPs. It contains revised IEP forms, effective January 2006, which are required for all special education students in Connecticut.

Colorado Department of Education, 2006 IEP Forms and Instructions
Colorado's IEP forms and instructions, available in English and Spanish.

Illinois statewide IEP forms
This is a file containing all of the IEP documents relevant to the individualized education program in Illinois. Included are: the Conference Summary Report, evaluation documentation, eligibility determination, present levels of academic achievement and functional performance, transition, functional behavioral assessment, behavioral intervention plan, documentation of accommodations, assessment documentation, manifestation determination, and annual goals progress report.

Massachusetts Department of Education IEP information
This page provides guidance relating to the IEP and implementing IDEA 2004 and includes: a side-by-side of the statutory language relating to IEPs and educational placements; key points for team meetings; a reference sheet for IEP team membership; key points for IEP development and progress reporting; an implementation guide; and a transition planning chart.

Michigan IEP manual
This IEP manual, revised in October 2007, includes information on the IEP and addendum, evaluation review, manifestation determination review, and interim alternative educational setting.

New Jersey IEP form
This is the individualized education program form for New Jersey special education students, revised in April 2007.

North Carolina IEP form
This IEP form for North Carolina special education students was revised in September 2007.

North Dakota IEP form
This is the individualized education program form for North Dakota's special education students, ages 6 through 15.

Ohio IEP Tool
IEP Tool, released in February 2006, which guides through the process of developing IEP. Further info about tool:
"OEC has developed a tool that provides examples of IEP statements with the opportunity to identify strategically designed statements that support improved student performance. The tool presents three sample IEP statements: 1) strategically designed to improve student performance, 2) minimally compliant and 3) not compliant. The user determines which label applies to each statement and the tool provides rationale for the correct response. OEC also developed an interactive version of the tool. The tool downloads as a compressed file and requires the end-user to extract the files on to a computer prior to running the program." [from website]

South Dakota IEP forms and guidance
On this webpage are the individualized education program forms for South Dakota special education students, revised April 2007, along with an eligibility guide revised in September 2007. 

Utah IEP forms
This webpage contains all individualized education program forms for students with disabilities, ages 3-21, in Utah. It includes forms for pre-IEP meeting preparations, eligibility determination by disability, ISP for private school students, observation reports, transition planning, and many other necessary forms.

Vermont IEP form
This is the individualized education program form for Vermont special education students, published in August 2007.

Virginia sample IEP form
This section of the Virginia Department of Education website offeres access to the IEP sample form and other related documents and information.
